- There is no charge to customers for flu shots (except as noted) if the customer brings an insurance card for any of the following: PacificSource Medicare, Samaritan Health Plan, Soundpath Health, PacificSource Community Solutions, Uniform Medical Plan of Washington, Samaritan Health Plan, Western Grocers Employee Benefit Trust, * Providence Health Plan, * LifeWise, Medicare Part B, Secure Horizons, Providence Health Plan Medicare HMO, Regence Blue Cross, Regence Life & Health, Regence Federal Employee Program, Regence MedAdvantage, Asuris Northwest Health, ODS Advantage PPO (medicare only), Sterling Option (medicare advantage), Certain HMO’s (bring card and ask nurse on duty). * Premera, * Group Health Cooperative.
